After you’ve had your second dose of a COVID-19 vaccination, the Health Department will automatically generate a digital vaccination certificate for you.

There may be times you’ll need to show evidence of your certificate to gain entrance to various establishments, including the The Barwon Heads Golf Club.

COVID-19 Vaccination Declaration (Accommodation Guest)

Members, guests, staff and contractors are required to show proof of being fully vaccinated (or a valid exemption) to enter the Clubhouse.

As a guest with an upcoming accommodation booking you are requested (pre-arrival) to email a copy of your vaccination proof or upload the same using the form below. All information will be kept at the Club under our strict privacy policy.

If you'd prefer not to upload your certificate, you will be able to prove your status on arrival; however, if you have forgotten your proof, or it is found to be invalid, you won't be able to access the Clubhouse and your accommodation booking will be cancelled with the appropriate cancellation fees charged.
